When something goes wrong, finding a quick fix can get operations moving again. But if you don't understand why the problem happened, there is a good chance it will happen again.
That's where Root Cause Analysis (RCA) comes in.

Effective investigations require teams to look beyond the immediate event.
What happened? Why did it happen? What conditions allowed it to happen? Could multiple causes have contributed?
Cause-and-effect thinking helps teams avoid stopping at the first plausible explanation and encourages a more complete understanding of the event.
Most importantly, the investigation shouldn't end when a root cause is identified. Corrective actions need to be implemented and monitored to determine whether the problem has actually been eliminated.
